Here is a situation that came to me 20+ years ago. 

I got a call from a couple that had just got married and on honeymoon on a small island in the Caribbean. It was a bad bite and the island did not have the medical facility to treat it. He needed to be medically airlifted to Fort Lauderdale. His insurance did not cover it. The company required $15,000 upfront to take him. They would not bill. 

I don't how it ended but I do know about shark bites. I had a house right off the beach in Daytona. There was a sign there.
